Texas Patient Rights Notice
1. Overview
In accordance with Texas House Bill 4224, Texas Health and Safety Code Section 181.105, and Texas Health and Safety Code Section 181.103, the following information is provided to help clients understand their rights related to access to health care records, professional regulation, and health information privacy.
2. Client Rights and Key Information
Requesting Access to Your Health Care Records
You have the right to inspect or obtain a copy of your health care records maintained by this practice.
Clients may also request that their records be provided in electronic format when available.
To request access to your records, submit a written request that includes:
- Your full name
- Your date of birth
- A description of the records requested
- Dates of service, if known
- Where the records should be sent
Requests may be submitted by one of the following methods:

Identity verification may be required before records are released. Records will be provided in accordance with applicable Texas law and federal privacy regulations. A reasonable, cost based fee may apply as permitted by law.

3. How to Request Records
Clients may request access to their health care records by submitting a written request using one of the methods listed above.
Requests will be processed in accordance with applicable Texas law and federal privacy regulations.
4. How to File a Complaint
Licensing Complaint
You may file a complaint regarding professional conduct with the Texas Behavioral Health Executive Council using the contact information listed above.
Health Information Privacy Complaint
You have the right to file a complaint regarding the privacy of your health information under Texas Health and Safety Code Section 181.103.
If you believe your health information privacy rights have been violated, you may file a complaint with the Office of the Attorney General of Texas:
