Texas Patient Rights Notice

Overview

Texas law requires covered health care entities to prominently provide detailed instructions explaining how consumers may request health care records, contact the applicable disciplinary or licensing authority, and file a consumer complaint concerning health information privacy. The information below is provided for clients of Pattern Shift Counseling, PLLC.


Client Rights and Key Information

Requesting Access to Your Health Care Records

You have the right to inspect or obtain a copy of health care records maintained by Pattern Shift Counseling, PLLC, subject to limited exceptions permitted by law. You may also request that records be provided in electronic form when available.

To request access to your records, submit a written request that includes:

  • Your full name
  • Your date of birth
  • A description of the records requested
  • Dates of service, if known
  • Where the records should be sent

Identity verification may be required before records are released. Records will be provided in accordance with applicable Texas law and federal privacy regulations. A reasonable cost based fee may apply as permitted by law.

You have the right to file a consumer complaint concerning the privacy of your health information under Texas Health and Safety Code Section 181.103. If you believe your health information privacy rights have been violated, you may file a complaint with:
