Washington Consumer Health Data Privacy Policy
1. Overview
This Washington Consumer Health Data Privacy Policy is provided to comply with the My Health My Data Act (RCW 19.373).
This policy applies to Washington residents whose consumer health data we collect. It is separate from, and in addition to, any HIPAA Notice of Privacy Practices and any general website privacy policy.

Categories of Consumer Health Data We Collect and Why We Collect It
We may collect the following categories of consumer health data and use them for the purposes described below.
Information you provide when you contact us or request services.
This may include information you choose to share through website forms, email, phone, or other communications that relates to mental health services, for example the reason you are reaching out, concerns you describe, and scheduling preferences. We use this information to respond to your inquiry, communicate with you, and coordinate requested services.
Appointment and service logistics.
This may include appointment requests, scheduling details, and communications related to providing services you request. We use this information to schedule, deliver, and manage requested services and to communicate with you about appointments.
Website and online interaction information that may indicate you are seeking mental health services.
This may include information such as pages you visit on our website, form submissions, device or browser information, IP address, and similar online identifiers when you interact with our website. We use this information to operate and maintain our website, improve website performance, and help maintain the security of our systems.

We collect consumer health data from the following categories of sources.

4. How to File a Complaint
If we deny your request, you may appeal our decision. If we deny an appeal, we will provide information on how to submit a complaint to the Washington State Attorney General.
